How to Report a Claim - Package, Auto and Umbrella

Reporting a claim can be completed in two ways:

By Fax: 800-953-8414
By Phone: 866-441-2641

Forms to complete when and injury occurs:

  • To report the claim by FAX, please complete eFAX Cover Sheet and fax to 800-953-8414. You will receive a confirmation by fax.
  • To report the claim by phone, please call 866-441-2641 and provide the details of the claim to the adjuster. To expedite the reporting process, please have all the information readily available before you make the call. The client number is 003470.

Correspondence after the claim has been reported:

  • Any other correspondence after the initial report should be sent to the Gallagher Basset Claims Office at P.O. Box 610, Roseville, CA 95678. Their phone number is 866-456-8402.

How to report a Claim - Workers Compensation

Reporting claims can be completed in two ways:
